We do not ask for and do not want your medical history, diagnoses, prescriptions, or Social Security number through these forms.
How we use it
We use your information to research health coverage options available to you, to contact you about those options, and to submit an application on your behalf if you choose to enroll. We may share your information with insurance carriers and their authorized representatives solely to obtain quotes or process an application you request.
We do not sell your personal information to unaffiliated third parties for their own marketing purposes.

If you provide your phone number and agree to be contacted, you consent to receive calls and text messages from Cantrell Health Insurance and its licensed agents at that number, including messages sent using automated technology.
